This document outlines the ‘terms of business’ between you and Centric Health with an address of Floor 7, RSA House, Dundrum Town Centre, Sandyford Road, Dundrum, Dublin 16, D16 FC92.

Terminology

The term Digital Doctor is the name of service provided by Centric Health. This is a service open to Irish Life Health policy holders.

The term 'doctor' or 'doctors' means a doctor or doctors working at Centric Health who are registered as medical practitioners with the Ireland Medical Council.

The term 'law' shall be construed as a reference to any law (including common or customary law), statute, judgment, regulation, directive, bye law, order, ordinance or any other legislative measure of any government, supranational, local government, statutory or regulatory body or court, in each case having the force of law.

Responsibility of our doctors for their patients

Our doctors take exactly the same personal responsibility for each patient as they would if they were seeing a patient face to face. The online nature of this service in no way diminishes the professional obligations that our doctors owe to a patient. The material and information available on the Centric Health website is intended for informational purposes only and does not constitute health or other advice nor should it replace face-to-face consultations with your own family doctor or medical specialist. Please consult a health care professional for specific recommendations about your condition.

The importance of providing true, timely and complete information

You agree that all information that you provide to our doctors will be true and complete to the best of your knowledge and that you will not purposefully omit to provide information that could reason-ably be judged to be potentially relevant to our doctors in providing the service.

 

Scope of services provided FOR DIGITAL DOCTOR

The service is provided to Irish Life Health Customers. Customer verification will be carried out by Irish Life Health .

Once verified you will be directed to https://digitaldoctor.irishlifehealth.ie.

A SMS or e-mail message will be issued to you to both confirm an appointment and if your appointment is cancelled.

You will receive a 15-minute reminder that your consultation is due to commence.

You agree to cancel an appointment at least 2 hours in advance of allotted time.

You will choose to take a phone consultation or video consultation. Video consultations are not available during out of hours service.

If you are late for your appointment you will receive an SMS message or telephone call to either rearrange or cancel the appointment.

Each consultation has an allotted time of 15 minutes.

An SMS or e-mail message will be issued to you if there are any delays in the service.

After the consultation you will receive a feedback form to enable Centric Health to improve services and to take a note of any trends where we may need to alter services provided to you.

 

Scope of services provided FOR FOR FEMALE HEALTH CONSULTATIONS

The service is provided to Irish Life Health Customers. Customer verification will be carried out by Irish Life Health.

Once verified you will be directed to https://femalehealth.irishlifehealth.ie.

An SMS or e-mail message will be issued to you to both confirm an appointment and if your appointment is cancelled.

You will receive a 30-minute reminder that your consultation is due to commence.

If do not attend your appointment, you will be charged for the consultation. If you are late for your appointment, no additional time will be made available.

Appointments cancelled 48 hours or more before the appointment is due to commence will be refunded.

Each consultation has an allotted time of 20 minutes.

An SMS or e-mail message will be issued to you if there are any delays in the service.

A post-consult plan will be sent to you within 24 hours of the consult ending via secure email to the email address provided by you at the time of booking.

After the consultation you will receive a feedback form to enable Centric Health to improve services and to take a note of any trends where we may need to alter services provided to you.

FEMALE HEALTH CONSULTATION REFUND POLICY

The specific refund policy for a credit or debit card can vary depending on the issuer and the terms and conditions of the card. However, there are some general guidelines that apply to most credit card or debit refund policies.

If you book an appointment with a credit or debit card and then decide to cancel the appointment, you may be entitled to a refund.

A refund will be due if you cancel the appointment 48 hours or more before the appointment and or if your payment has yet to be processed.

It's important to note that it can take 3 to 4 working days for the refund to appear on your credit or debit card, depending on the policies of your provider or bank.

 

Privacy Policy and patient confidential data

Centric Health has a legal obligation to protect all patient confidential data. We are not allowed to share patient confidential data with third parties, who are unconnected to our service without your explicit consent. All patient confidential data is stored electronically on servers owned and managed by Centric Health . Full details can be located here

Declaration

By agreeing to our Terms and Conditions and using the site, you declare you wish to take part in the online medical consultation service operated by Centric Health.

You undertake and warrant that all information provided by you is correct, true and complete.

Prescriptions may be issued in accordance with information you provide to our doctors through assessments. We rely on the information provided by you and therefore accept no liability for loss or damage arising from our service or medication prescribed if you supply incorrect or incomplete in-formation.

You agree to promptly inform our doctors of any side effects of treatment and any other issues arising from treatments provided.

Centric Health will make every effort to issue an update and record of notes or prescription to your own GP . Centric Health are not liable for any damage which may arise from failure to inform your GP or other healthcare professional about treatment you receive. It is therefore advisable that you advise your GP if there are any recommendations made or prescription issued.

You accept the advice that is provided by Centric Health does not replace your GP and that you should consult with your GP and other health carers when you are advised by our doctors and as need arises.

You agree that any medicine our doctors prescribe to you will be exclusively for your own personal consumption and not for any other person and you undertake not to supply any other person with any such medicine. You agree that you will read the manufacturer’s Patient Information Leaflet that

accompanies the medicine and that you will follow any advice that the dispensing pharmacist provides to you.

You give your consent for information about yourself, your health and your medical history including, but not limited to, the information you divulge as part of the online consultation assessment to be viewed and exchanged by and between Centric Health (Digital Doctor team) employees and a Doctor working on Digital Doctor’s behalf for the purpose of conducting a medical consultation. You understand this information may be exchanged electronically.

 

Change to the Terms and Conditions

By using the service, you agree to be bound by these Terms. We reserve the right, in our sole discretion, to change, modify, add or remove portions of these Terms at any time
